- Coordinate services needed by each participant, across agency lines
- Creates Individual Service Plans (ISP) unique to each participants needs
- Serve as the single point of contact in helping participants and their families to obtain the support and services they need
- Monitors the health and welfare of the participant on a regular basis
- Facilitates referrals to additional agencies, both waiver and non-waiver, if appropriate
- Identifies available providers
- Monitors the delivery of services
- Evaluates and assesses participant Individual Service Plans (ISP)
- Advocates on behalf of the participant for additional support and services as needed
- Assists in identifying and sustaining a participant’s natural support system including family, friends and associates
