Stokes strike with very little warning and result in several neurological complications. This, in turn, changes the way a person’s body functions and profoundly affects life as they know it. Depending on which side the stroke occurs, there may be a change in speech, memory and/or vision.
Given that no two strokes are the same, the long-term and short-term effects of this complex condition can be difficult to navigate. The goal is to provide services to help individuals cope and adjust to their life a stroke.
Care Lync Coordinators help Medicaid-eligible adults who have had a stroke navigate the OBRA Waiver program. From environmental adaptations to daily living services, we help waiver Participants get the care they need in order to return to a life of independence, self-worth, and dignity.

Care Lync provides service coordination for adults diagnosed with Physical, Intellectual and Developmental disabilities who are enrolled in the Department of Human Services Medicaid Waiver Programs. Since each waiver has its own unique set of eligibility requirements and services, our Coordinators guide Participants to the resources and available options. Our passionate team of seasoned experts help ensure that our Participants get the care they need in order to lead a healthy and independent lifestyle in their home and community.
