Learning to live with this long-term condition can be difficult to navigate. Though Osteoarthritis can affect each person differently, the symptoms and subsequent lifestyle adjustments is a challenge – especially if one doesn’t have the right support systems and care in place.
Depending on the level of care needed, Care Lync Coordinators help Medicaid-eligible adults with Osteoarthritis navigate the Attendant Care Waiver program and additional resources through the Independence Waiver. Our expert team helps Participants access the community-based services available to them. From daily living assistance to non-medical transportation, we help waiver Participants get the care they need in order to return to a life of independence, self-worth, and dignity.
