Millions of people are diagnosed with COPD – still, many more don’t even know they have it. This chronic lung disease, which makes it hard to breathe, is a slow progressing disease. Symptoms can become more severe over time, making daily activities such as cooking or walking difficult.
No matter how they present, the long-term effects of this chronic disease can be difficult to navigate and the impact on the person afflicted and their family is unimaginable. The goal is to provide a personalized action plan to reduce risk factors, manage stable COPD, treat acute exacerbations and manage associated illnesses.
Care Lync Coordinators help Medicaid-eligible adults with Chronic obstructive pulmonary disease (COPD) navigate the Attendant Care Waiver program and additional resources through the Independence Waiver. Our expert team helps Participants access the community-based services available to them. From assistance with performing activities of daily living such as bathing to home delivered meals, we help waiver Participants get the help they need in order to feel better, stay more active and slow the progress of the disease.
